What are your policy and procedures?

Policy and Procedures

Upon receipt of referring physician’s order, the patient is to be evaluated by a certified practitioner. Should any written order lack sufficient detail, or functional level, appear duplicative or medically unnecessary, the physician must be contacted and the problem clarified.

Upon initial contact with the patient, the patient treatment plan with regards to O&P services is to be fully explained to the patient. This is to include but not be limited to procedures used, time frame involved, estimated cost, and potential outcomes. All questions pertaining to O&P management are to be addressed fully. With the patient’s consent, initial casting and measurements are then taken.

After fabrication of the device the patient will be fitted and the device statically and dynamically aligned. All skin pressures are evaluated and adjusted as needed. When all appears satisfactory the device will be finished and delivered.

At final delivery, patient is to be re-fit with the device to ensure that no minor adjustments are needed at that time. The patient will be asked to sign a receipt of item. The patient is then referred back to the prescribing physician for final medical evaluation and approval of the device.

If a patient is currently in a Physical Therapy program the therapist is to be contacted and apprised of the patient status.

Follow Up Procedures

In all cases: Patients are to be instructed that if there are any problems, questions, or complaints of any sort, they must contact the office so that the problem or concern may be properly addressed.

All complaints will be initially responded to within two business days; except in the case of emergency repairs which will be addressed within the next business day or sooner if possible.

If our office cannot satisfactorily resolve a beneficiary’s complaint, the patient is to be referred back to the prescribing physician for resolution.

Choice Limb and Brace is by appointment only. However, we strive to accommodate our patients at their home, Doctor’s office,  Physical Therapy center, or Clinic. Call us at (914) 479-0743 or (888) LEG-BRACE to make an appointment and we will do our best to accommodate your schedule.
